3. Counter-Notice Procedure

• Identification of the material that was removed and the location where it appeared.
• A statement under penalty of perjury that you have a good-faith belief the material was removed by mistake or misidentification.
• Your name, address, and telephone number, and a statement consenting to Irish or EU jurisdiction.

4. Repeat Infringers

IPTV Ireland's policy is to terminate, in appropriate circumstances, the accounts of users who are repeat infringers of intellectual property rights.
